New Subaward and Vendor Agreement Forms Online

Sponsored Programs is excited to share with you that requests for Subawards and Vendor Agreements may now be submitted online!

The following forms are now available for online submissions: 

1.     Subaward Request Form

2.     Subaward Amendment Request Form

3.     Vendor Agreement Request Form

4.     Vendor Agreement Amendment Request Form

These forms are also linked from the Forms page of our website.  Prior to submitting an online request, you will need to prepare the required attachments (available from the Forms page) as separate Microsoft Word documents.  You will be able to attach these files to the online form.  Please note that the form will prevent submission unless all required fields have been completed; required fields are marked with a red asterisk.

To accommodate those who may have requests in process, we will accept previous versions of the request forms through July 31.  Beginning August 1, all requests must be made using the new online submission process. 

Over the next few months, we will be working toward implementing additional online submission processes.  Please watch our website for updates.
