Budget Revision Request Form for Internal Funding Awards

Budget Revisions may be requested for UFS Mini Grants, UFS Major Project Awards, and Scholarship on Teaching Awards using this form.  An updated copy of the Detailed Budget Form must be attached.  Please update the budget form previously submitted or download a new copy for the program under which your award was made (Mini Grant Detailed Budget; Major Project Award Detailed Budget).  

    Please provide a summary of your research and activities that have been completed to date. 

    Please provide a justification for the changes you are requesting to make to the budget, including why new new costs are necessary to complete the project.  

    Attach a copy of the updated Detailed Budget Form with the changes reflected.
